Applications for re-issuance of a MOFA ID or BTE Card in cases of loss or theft shall be submitted by a note verbale or official letter enclosed with the following documents:
1. A copy of the police incident report;
2. A completed Application Form for Identification/Business Tax Exemption Card;
3. A copy of valid passport;
4. A copy of valid visa;
5. A recent 1-inch photograph on a white background.
